ClinCard Human Subject Instructions

• How Can The ClinCard Be Used:
  • • At any merchant that accepts Visa.
    • For online purchases.
    • To transfer available card balance to your bank account using the My ClinCard website or mobile app.
• Inactivity fee:
  • • A fee will only be deducted from a card if the card becomes inactive and there are still available funds on card balance.
    • Card inactivity is defined as no funds being applied to or removed from the card for 6 consecutive months.
• Cash withdrawal options:
  • • Cash Advance
      • Visit any local bank to receive a cash advance off the card. Fees may apply, this is decided by the bank.
      • Your government ID and the address registered to the card will be required for the withdrawal.
    • ATM Withdrawal
      • • You will not be charged any additional convenience or surcharge fee at a Fifth Third Bank ATM.
        • Use this link (https://locations.53.com/search.html) to search for the location of all Fifth Third Bank ATM’s.
        • Type in the zip code of your location
        • 
        • Uncheck BRANCHES and PARTNER ATMS. Only have Fifth Third ATMS checked. This will give you location that will not charge a fee.
